Helping to care for a loved one is a rewarding experience. Sometimes it can also be a little stressful. Superior HealthPlan understands and we are here to help you.

Superior offers Service Coordination/Care Management to support caregivers. These teams have Service Coordinators and Care Managers who can help. Service Coordinators/Care Managers can give you the extra support you may need as you assist others with:

  • Personal care
  • Emotional support
  • Medical care
  • Education
  • Feeling safe and secure

Through useful tools and information, we can help you care for our members, while also continuing to care for yourself.
