I am a Superior Member- My Account

You should receive your Superior HealthPlan ID card in the mail once you are enrolled with Superior.

If you did not get this card, or you need a replacement:

  • Call Member Services for assistance.
  • Register and access our secure member portal. From there you can request, print a temporary ID, or download a digital version of your ID card on your smart phone.

You can locate your member ID number on your member ID card. If you misplaced your ID card, please call Member Services for your Member ID number and new ID card.

Superior HealthPlan can make updates to your phone number on file. We cannot change your name, address, date of birth, Medicaid plan or gender.

To update your information, call Texas Health and Human Services (HHS) to report any changes. HHS will update your Medicaid file. To update your personal information:

Before you contact Superior HealthPlan to report changes, please contact HHS at 2-1-1. Dialing 2-1-1 is free. If you cannot connect to 2-1-1, please call 1-877-541-7905.

Please note, changes to your Medicaid case can take up to 15-45 days. You can always call Member Services to let them know of your change of address. They will note your new address on your account, while HHS officially updates your address in their records.

Due to the sensitivity of this information, you will need to:

  • Call Member Services to assist you with this request.
  • Register and access your account through our secure member portal. On the portal, you will be able to:
    • View your health history 
    • Update personal information (after calling 2-1-1)
    • Print a temporary ID card
    • Find and update primary care provider (PCP)
    • Review your health status
    • Send Superior a secure message
    • Check the status of authorization requests
    • Review Claims and Explanation of Benefits
    • See who is your Service Coordinator (if applicable)
    • Check rewards balances
    • Complete a wellness assessment(if applicable) 
    • View Individual Service Plan (for STAR Kids)* 
    • Find health education information
    • View Screening and Assessment Instrument
      (for STAR Kids)*

You can use Superior’s Find a Provider tool to search through our directory of Primary Care Providers (PCPs), specialists, hospitals, pharmacies, and other health-care providers.

Due to the specific information required to make this change, you will need to:

Superior HealthPlan offers telehealth services to all our members. Telehealth is a virtual health-care visit with a provider through a mobile app, online video or telephone. This service provides 24-hour access to in-network health-care providers for non-emergency medical issues.

If you need assistance with your registration, please contact Member Services.

Depending on your coverage, you may be eligible for non-emergency transportation to health-care appointments, follow-up appointments, dentist appointments, hospital, pharmacy, and other places you may get services.

Congratulations on your new addition! To submit notification of your newborn baby, contact Texas Health and Human Services (HHS). You have two options to contact HHS:

Once you have notified HHS, you call Member Services and they will add a note to your file and answer any additional questions you may have.

Your coverage may offer dental benefits. If you selected a dental provider during enrollment, please contact them for detailed dental benefits information:

To get specific information about a member or benefit, please contact Member Services.

  • Medicaid ID number
  • Member Name
  • Date of Birth
  • Social Security Number
  • Address and phone number

Member Secure Portal

To register your portal account, you will need your date of birth or your member ID number (found on your Superior member ID card). For more information, please click Superior’s Member Portal Login webpage.

If you need further assistance with your portal registration, please contact Member Services.

If you are trying to create a new member portal account or accessing an old account, and the information you are entering is not recognized, please contact Member Services to help.

In order to reset your member portal password, please click the “Forgot password” option on the Member Portal Login webpage. Please note: Your User Name to log in is your email address.

I am a not a Member

You can get detailed plan information by contacting your local HHS office at 2-1-1. Or by visiting the Get Insured through Superior HealthPlan webpage.

After you’re approved for Medicaid, you’ll get an enrollment packet in the mail. You need to choose a health plan or the enrollment broker will choose one for you. Call 1-800-964-2777 (TTY 711) to join Superior.
