Care Management

Superior HealthPlan Care Managers are available to assist members with medically or socially complex needs who may benefit from increased coordination of services to optimize health and prevent disease. The Care Management Department is staffed by individuals with extensive medical or social work experience in areas such as obstetrics, oncology, behavioral health, medical/surgical, HIV/AIDS and physical rehabilitation for children and adults.

Early intervention is essential to maximize treatment options while minimizing potential complications associated with catastrophic illnesses or injury and exacerbation of chronic conditions. A Care Manager is available between 8:00 a.m. – 5:00 p.m. (CST) to assist with coordination of the members’ health–care needs.

The following conditions and/or diagnoses are examples of appropriate referrals to the Case Management Department:

  • Asthma
  • Diabetes
  • Heart Disease
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure
  • Sickle Cell
  • Transplants
  • Maternal Health, Children and Pregnant Women (CPW)
  • Mental Health and Substance Misuse
  • Non-Medical Drivers of Health (social isolation, hunger, housing, domestic violence)

The provider’s role in Superior’s Care Management program is extremely important. Practitioners who have identified a member who they think would benefit from disease or case management should submit a referral request using the Superior's Secure Provider Portal or by contacting the Care Management Department at 1-855-757-6567:

  • Option 2 – STAR/CHIP 
  • Option 3 – STAR+PLUS 
  • Option 4 – STAR Health 
  • Option 7 – STAR Kids
