Effective 12/27/24: OnTrak Partnership Ends

Please note: This is an update from the previously posted article: Effective May 1, 2021: Updates to Ontrak Program Service Claims and Appeals.

Superior HealthPlan’s agreement with OnTrak will be ending and contract amendments, which include provisions for OnTrak program services, will no longer be valid effective December 27, 2024.

Important notes for providers billing for dates of service on or after December 28, 2024:

  • Claims will no longer be eligible for reimbursement according to the rates outlined in the OnTrak contract amendment.
  • Providers should no longer bill the X2 modifier in the secondary position, indicating OnTrak program services.
  • Reimbursement rates will follow standard Medicaid fee schedules and/or the compensation schedule outlined in the standard participating provider agreement with Superior.
