Billing Requirement Change: Claim Billed on UB-04/CMS-1450

Effective August 31, 2020, Superior implemented new billing requirements for all providers billing on UB-04. In order for claims to be accepted, providers must have a different National Provider Identifier (NPI) listed for rendering and billing. Please ensure a different NPI is listed in box 56 (NPI) for rendering and box 76 (attending NPI) for billing on UB-04/CMS-1450 forms, which can be found by visiting the Centers for Medicare & Medicaid Services (CMS) website.
