  • Prior to a new provider’s provision of services and receipt of claims reimbursement, program enrollment must be confirmed on requests for Superior Medicaid (STAR, STAR Health, STAR Kids and STAR+PLUS), CHIP and/or Long-Term Services and Supports (LTSS) programs.

  • Superior will notify you of acceptance or non-acceptance to our network no later than 90 days from receipt of a complete application for participation.
