REFER MEMBERS: Disease Management Program

Superior HealthPlan is committed to improving the quality of care delivered to members living with chronic illness through our comprehensive Disease Management (DM) programs. DM is an innovative service delivery approach designed to provide specialized care to members with chronic illness.

DM programs can help improve member health by minimizing and preventing the effects of disease through integrated care. Focusing on integrated care aids in the reduction of health-care costs. Successful DM programs result in fewer doctor visits and help to eliminate trips to the hospital and/or emergency room.

In an effort to educate and help members manage their chronic health conditions, DM programs use a team of multidisciplinary providers which can include:

  • Physicians
  • Nurses
  • Pharmacists
  • Behavioral health providers
  • Radiation therapists
  • Respiratory therapists
  • Psychologists
  • Dieticians

Please note: Providers may also choose to work with support-service providers to fill gaps within the DM care team.

Superior’s parent company, Centene, provides DM services for Superior members, and is dedicated to helping them improve their health and well-being through coordination with both the member and their providers to focus on disease-specific conditions. Vida Health is the DM vendor that assists with providing services for Ambetter from Superior HealthPlan members. Vida Health offers health coaches, nutritionists, diabetes educators, nurses, trainers and licensed therapists to provide guidance and supportive accountability to members.
