REQUIRED: Resident Transaction Notices for Nursing Facilities

Superior HealthPlan would like to remind STAR+PLUS Nursing Facility providers that they are subject to all reporting and notification requirements for Resident Transaction Notices (Forms 3618 and 3619).  

Nursing Facility providers are responsible for notifying Texas Health and Human Services Commission (HHSC), the Texas Medicaid & Healthcare Partnership (TMHP), and Superior after a STAR+PLUS member’s admission or discharge from a Medicaid Nursing Facility and when Medicaid applicants and recipients have a significant change in condition.
