Superior HealthPlan & Prospera: Addressing Social Determinants of Health

Social determinants of health are conditions in the places where people live, learn and work that affect overall health. This includes access to food, transportation and affordable housing, all of which can significantly affect a person’s overall well-being. Since 2018, Superior HealthPlan and Prospera Housing Community Services have partnered to address these social determinants, and are making a real impact in the lives of thousands of people across South Texas. A new video explores their collaborative approach to transforming the health of Texas communities, one person at a time.
