IMPORTANT: VDP COVID-19 Updates Effective 1/1/25

Effective January 1, 2025, in accordance with the Texas Vendor Drug Program (VDP), Superior HealthPlan will no longer pay pharmacy claims for COVID-19 at-home test kits using the following “dummy NPI" and practitioner: 

  • NPI: 3070440003
  • Provider Last Name: Test Kit Prescriber
  • Provider First Name: COVID

All at-home test kits will require a prescription from a licensed prescriber to dispense under Superior benefits. 

Additionally, in accordance with the VDP, Superior will no longer cover COVID-19 vaccines for members 18 and younger. COVID-19 vaccines will be covered under the Vaccines for Children (VFC) program.
