IMPORTANT: Updates to the Provider Statement of Need Form

Superior HealthPlan would like to inform providers about updates made to our Provider Statement of Need (PSON) form for STAR Kids and STAR Health. These updates include the addition of following form fields:

• Member Height
• Member Weight
• Date of most recent Texas Health Steps/HEDIS visit

Please Note: These new fields are not a requirement and will not delay the PSON process if they are not included on the form.
