ACTION REQUIRED: Rate Enhancement Affidavit

Providers who are currently participating, or would like to participate, in the Superior HealthPlan Rate Enhancement program for STAR+PLUS, STAR+PLUS Medicare-Medicaid Plan (MMP) and STAR Kids (Day Activity and Health Services [DAHS] only) are required to complete and submit an affidavit annually to enroll and/or continue their participation. This affidavit affirms that any amount paid through the Rate Enhancement program has been applied to the compensation for direct-care staff.
To enroll or continue participation in the program, the affidavit must be signed and returned by October 31, 2024. Please note:
- If the affidavit is returned by October 31, 2024, claims from September 1, 2024 will be reprocessed to reflect levels awarded.
- If a provider fails to submit their affidavit timely, they will be removed from participation in Superior’s Rate Enhancement program effective January 1, 2025.
Providers can submit their Rate Enhancement affidavit through Superior’s Rate Enhancement Affidavit online form.
Please note: The following programs are eligible for Rate Enhancement: Primary Home Care (PHC), Day Activity and Health Services (DAHS) and Assisted Living.
